1. Click on the form you want to fill out.
2. Fill out the form.
3. Save a copy for your files.
4. Send as an attachement with an email to the Church Planting Department at scassell@rmdc.org.

Monthly Church Report Save the Microsoft Excel form on your hard drive.  After it is completed, send it as an attachment with an email to scassell@rmdc.org.
